2022-2023 COVID Policies and Procedures

Updated (August 24, 2022)

Masks/Facial Coverings:

As of now, face masks are OPTIONAL for all dancers, parents and staff.

Health Monitoring:

Students must take temperature before coming to the studio. If they are displaying symptoms that could be attributed to COVID-19 (cough, difficulty breathing, sore throat, unusual headache, unexplained muscle or joint pain, chills, fever, nausea, vomiting, diarrhea, loss of smell, pink eye, fever, etc.) they are asked to stay home and take advantage of our make-up class policy. If a dancer becomes ill during class the parent will be called immediately and asked to pick her/him up ASAP.

Studio Drop-Off/Pick-Up Procedure:

Please do not arrive for class more than 5-10 minutes before the start of class. Parents are encouraged to drop-off/pick-up, and not linger in hallways or waiting areas, so that we have more space for our students.

All dancers must check in at our attendance desk before entering their classrooms.

Cleaning and Sanitizing Procedures Daily and Between Classes:Hand Sanitizer will be provided in every room, bathroom and hallway.  Medical grade HEPA Purifiers have been purchased for each studio, waiting area, and hallway.  Daily cleaning/disinfecting of studios, bathrooms, lobbies, and other high touch areas will continue.

SAFETY is our priority for all of our staff and students.  We will continue to monitor guidance from the Governor, CDC, and the Needham Board of Health, and will adjust our policies as needed

2022-2023 Waiver/Studio Policies

Updated (August 24, 2022)

COVID-19

I have read and agree to abide by the Total Eclipse Dance Studio COVID-19 Safety Policy and Procedures as outlined on our website. I understand that Total Eclipse Dance Studio strives to provide the safest environment possible and minimize health risks. However, I also understand that there is an inherent risk of exposure to COVID-19 and other illnesses/disease in any public place where people are present, which includes participating in classes at Total Eclipse Dance Studio. I understand that I am fully and personally responsible for my own safety, and the safety and actions of my dancer/child and/or immediate family members while at or on the property of Total Eclipse Dance Studio, located at 1132 Highland Avenue, Needham, MA, and I recognize that I and my dancer/child and/or immediate family members may be at risk of contracting COVID-19 and potential other illnesses/disease.

I understand that if a student, faculty or staff member tests positive for COVID-19, parents with dancers enrolled in the same classes will be notified in a timely manner. I understand and agree that in the event that any/all in-studio classes are converted to virtual classes, for any reason, tuition will not be refunded or credited.

With full knowledge of the risks involved; I hereby release, waive, and discharge Total Eclipse Dance Studio, its owners, staff, independent contractors, affiliates, employees, representatives, and successors from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by myself, my dancer/child, or immediate family members related to COVID-19 while participating in any activity while in, on, or around the premises while using our facilities at 1132 Highland Avenue, Needham, MA, that may lead to unintentional exposure or harm due to COVID-19.

Tuition

I understand that there are no tuition refunds for classes; this includes classes your dancer is scheduled to attend but is absent from, classes that are canceled due to inclement weather or any unforeseen circumstances such as a health crisis/pandemic, and any class(es) that your dancer does not participate in due to injury, illness or any other condition. I understand that there are no refunds for registration fees, costumes fees (including deposits/balance), performance tickets, recital fees, or rehearsal fees. Tuition is due in two installments as per the dates indicated in the current season calendar. If your payment is not received within 14 days of its due date, you will be charged an additional $35 per month that the balance remains unpaid. Any exceptions to the refund rule can only be made by the Directors of Total Eclipse Dance Studio:  Gemma Joyce and Alicia Santamaria.

I understand that dancers and families will not be able to enroll in classes, attend classes, or participate in any performances, if there is an outstanding balance on their account. This includes outstanding payments from previous dance seasons.

Withdrawal Policy

There are no refunds after the first three classes. After the third class, a credit may be issued and can be used for classes within the current or upcoming dance studio season. If after one month no request in writing has been made, no credit will be issued (except with injury; a doctor's note must be provided). Registration fees, costumes fees (including deposits/balance), performance tickets, recital fees, and rehearsal fees are non-refundable and will not be credited regardless of withdrawal date.

Release of Liability

I understand that dance classes may include, without limitation, strength and conditioning, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Total Eclipse Dance Studio strives to reduce this risk through proper dance training and technique. Each student may decline to participate in any activity which she/he feels may be harmful and is responsible to inform the instructor of any physical limitations which may prevent full participation in class. Total Eclipse Dance Studio and its employees, instructors, subcontractors, volunteers and participants are not responsible or liable for personal injuries or damage to physical property. This Release Of Liability includes all classes held by Total Eclipse Dance Studio, Needham, MA and also any class(es) hosted or taught by Total Eclipse Dance Studio using an online, virtual platform, (such as ZOOM) that remotely/virtually teach dancers.

Medical Emergency

I give permission to Total Eclipse Dance Studio and it's owners and operators the consent to any reasonable medical treatment deemed necessary by a licensed physician in the event that the participant is injured while at Total Eclipse Dance Studio and we are unable to reach a parent or guardian. I also acknowledge that I will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by the health insurance carrier. I/we agree that Total Eclipse Dance Studio will not be responsible for any medical care or costs that are incurred. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

Medical History

I hereby declare any and all physical/mental problems, restrictions, allergies or conditions, are listed within this registration form, and/or declare student(s) to be in good physical and mental health and able to participate fully in dance classes without any restrictions.

Photography Consent

Total Eclipse Dance Studio uses photography on our website, newsletters, social media, videos, brochures and other marketing material. I agree that Total Eclipse Dance Studio may use my child/dancers photo (from class photos, recital images etc.) for these purposes unless Total Eclipse Dance Studio is notified, in writing via email to totaleclipsedancestudio@gmail.com, that we may not. Students are free to refuse to be photographed.
